Please only complete this form one time per participant. This form completes the membership registration process and is required prior to participating in any events hosted by The NEST for All. There is no fee associated with membership. Completing this form does NOT register you for any events hosted by The NEST for All. Event registration can be found on the EVENTS page of our website, www.TheNESTforAll.com.
We’re committed to creating spaces where all participants can thrive with the appropriate supports in place. Members are encouraged to bring any tools or supports they use in daily life. Our events are safe and lightly supervised to encourage independence and natural social interaction. We want everyone to have a safe, fun, and respectful experience where they can grow and connect, and truly feel welcomed, valued, and a sense of belonging in The NEST. One-on-One Support
While we strive to be welcoming and inclusive, we do not provide personal, medical, or behavioral support. Participants who require one-on-one support — for any reason, including medical needs, behavioral support, mobility, or the safety of themselves or others — are required to attend with a caregiver or support person.

What you can expect of The NEST for ALL
Staff at The NEST for All WILL:
- Lightly supervise to ensure the safety and welfare of the group
- Use teachable moments to educate and empower members
- Have conversations and use strategies to support communication, socialization, self-determination and empowerment
- Support members in managing use of technology to ensure it’s used responsibly and constructively.
Staff at The NEST for All LLC WILL NOT:
- Provide individual care. We are not responsible for monitoring allergies, insulin, medication, food/drink intake, toileting, or any other activity that requires personalized attention
- Provide progress updates, data collection, observations, or written statements or reports
- Create lessons or provide formal or individualized instruction

Member Responsibility
I understand that any tools, supports, materials, personal belongings, or supplies I bring to The NEST for All LLC events are my responsibility to keep track of, and The NEST for All LLC is not responsible for their loss, damage, or replacement.

Cancellation Policy
Staffing at The NEST is determined based on the number of registered participants. Should a member be unable to attend an event they have already registered for and paid for, then the following cancellation policy applies: Cancellations made more than 24 hours prior to the event start time will receive a full credit equal to the event fee, applicable to a future event. Cancellations made within 24 hours of the event start time will receive a 50% credit toward a future event.

Image and Likeness Release
I hereby grant The NEST for All, LLC and its agents, photographers, and filmmakers, permission to use my image and likeness for promotional and editorial purposes. This includes, but is not limited to, use in still photography, video, and film, for publication or distribution in newspapers, magazines, websites, social media, television, and video productions. I waive any right to inspect or approve the finished product or editorial layout that may be used in connection therewith. I further waive any right to monetary payment or compensation, now and in the future, for the use of my image and likeness.

Arrival, Departure, & Pick-Up Policy
To ensure the safety and smooth operation of all events hosted by The NEST for All, the following policy applies: All participants are required to check in upon arrival and check out before leaving each event. Participants (or their designated caregivers/guardians) agree to be picked up promptly at the scheduled end time of the event. A late fee of $15 will be charged for every 15-minute period past the scheduled pickup time. Charges apply in full for any part of a 15-minute period.

Consent Statements
I understand and agree that engaging in any business activities, commercial transactions, or the exchange of money is strictly prohibited at any events hosted by, or within any spaces owned, leased, or occupied by, The NEST for All LLC, unless prior written authorization has been granted by an authorized representative of The NEST for All LLC. I acknowledge that any violation of this policy may result in immediate removal from the premises and/or termination of membership or participation privileges.

I understand and acknowledge that parents, legal guardians, members, and staff members of participating groups are fully responsible for the conduct and behavior of the member(s) under their supervision at all times. Such individuals are expected to monitor, manage, and appropriately address any behavior that is deemed inappropriate, noncompliant, disruptive, or otherwise questionable, in accordance with the standards and policies of The NEST for All, LLC.

I understand and acknowledge that The NEST for All, LLC reserves the right, at its sole discretion, to deny, suspend, or terminate my participation or membership at any time, for any reason, with or without cause or prior notice. I further acknowledge that such action may include exclusion from programs, events, or activities offered by The NEST for All LLC, and that no refund or compensation shall be due in the event of such termination or exclusion unless otherwise required by law.

Comprehensive Waiver of Liability, Release, and Consent to Medical Treatment
In exchange for my being allowed to participate in The NEST for All LLC programs and activities, I, and if I am not my own guardian, my legal guardian, agree to be bound by each of the following:
Identification of Risks.
I understand that participation in the Program may involve risk of injury, disability or death. Assumption of Risks. I assume all risks connected with my participation in the Program. I accept personal responsibility of any liability, injury, loss or damage in any way connected with my participation in the Program. Waiver of Release. I release and discharge The Nest for All LLC, and each of their affiliated organizations, directors, officers, sponsors, employees, agents, successors, and assigns from all claims for any liability, injury, loss, or damage in any way connected with my participation in the Program. I acknowledge that the Program is not liable for injury arising out of participation in the activities, even if caused by the ordinary negligence or otherwise of The Nest for All LLC, and each of their affiliated organizations, directors, officers, sponsors, employees, agents, successors, and assigns. I intend for this waiver and release to also apply to my relatives, personal representatives, heirs, beneficiaries, next of kin, and assigns who might pursue and legal action or claim for such liability, injury, loss or damage.
Consent for Medical Treatment.
I agree that The Nest for All LLC may, but have not duty to provide me, through medical personnel of their choice, medical assistance, transportation, and emergency medical services. In the event that The NEST for All LLC must seek medical treatment for a member or a volunteer, The NEST for ALL LLC is not responsible for any fees or expenses incurred for medical treatment. All expenses are the responsibility of the member.
Hold Harmless.
I agree to indemnify and hold harmless The Nest for All LLC for all claims arising out of my participation in the activities. Health and Lack of Impairment. I, or my parent/legal guardian, represent that, to my/their knowledge, I am in good health and suffer no physical impairment that would or should prevent my participation I understand this waiver is intended to be as broad and inclusive as permitted by the laws of the state of Michigan and agree that if any portion of the agreement is invalid, the remainder will continue in full legal force and effect.

Anti-Discrimination & Anti-Harassment Policy
Our Commitment At The NEST for All – We are committed to creating and maintaining a safe, respectful, and inclusive environment for people of all abilities, backgrounds, and identities. Discrimination, harassment, or any form of mistreatment will not be tolerated. We celebrate diversity and are dedicated to fostering a culture of belonging, where everyone—regardless of ability, race, ethnicity, gender, sexual orientation, religion, age, or background—can participate fully and comfortably in our community.

The following behaviors are strictly prohibited at all NEST events, programs, and locations:

Harassment
Any behavior that creates a hostile, intimidating, or offensive environment, including but not limited to:
- Verbal abuse, slurs, or derogatory comments
- Physical intimidation or unwanted contact
- Inappropriate jokes, gestures, or visuals
- Sexual harassment (e.g., unwelcome advances, comments, or touching)
- Bullying or repeated demeaning behavior
- Expectations for Behavior
All members, staff, volunteers, and visitors are expected to:
- Treat others with dignity and respect at all times
- Use inclusive, respectful language
- Respect personal boundaries and consent
- Report any observed or experienced instances of discrimination or harassment
- Engage in good-faith communication and conflict resolution when appropriate
- Reporting & Response –
If you experience or witness discrimination or harassment:
- Report the issue immediately to a NEST staff member
- Reports may also be submitted confidentially via email at TheNESTforAll@gmail.com
All reports will be taken seriously and responded to promptly. The NEST will:
- Conduct a fair and thorough review
- Maintain confidentiality to the extent possible
- Take appropriate corrective action, which may include warnings, mediation, suspension, or removal from the community
Retaliation for reporting concerns is strictly prohibited and may result in immediate dismissal from the program or staff team.

- Policy Acknowledgment
All staff, volunteers, and members must read and agree to this policy prior to participation. By engaging with The NEST for All, you acknowledge your commitment to fostering a safe and inclusive space for all.
Registration & Payment Acknowledgment
Each event hosted by The NEST for All requires individual event registration and payment made through the Events page at www.TheNESTforAll.com. (This must be completed for each event.)

Signature
By signing below, I confirm that I am the participant, or if the participant is a not their own guardian, that I am the participant’s parent or legal guardian, and that I am authorized to provide this consent on their behalf. I have read, fully understand, and agree to be bound by all terms and conditions set forth in this Waiver, Release, and Consent Agreement. I understand that this document affects my legal rights and the legal rights of the participant, and I sign it voluntarily and of my own free will.
